Our group is conducting a mini-survey in line with our research study entitled “JOURNEY
TO BODY POSITIVITY: Exploring the Motivation and Challenges of Selected Senior
High School Students to Body Figure Improvement”. This study aims to spread
awareness about body image and its effect on mental and physical health. This study will
help you and others learn to embrace and love their bodies.

We would like your cooperation by answering the questionnaires wholly and honestly. Any
information that you will provide will be highly appreciated and we assure you that all

We are student researchers from Grade 12 - St. Alphonsus Liguori who are currently
conducting a research study entitled “JOURNEY TO BODY POSITIVITY: Exploring the
Motivation and Challenges of Selected Senior High School Students to Body Figure
Improvement”. Our study requires the administering of an interview form from which we will
derive answers to our research problem.

With this, we would like to request your voluntary cooperation to be one of the interviewees.
As a respondent, you are expected to answer the interview guide questions.

Be fully aware that the interview will be audio-recorded, but rest assured that your responses
to the interview questions will be treated with the utmost confidentiality and personal
information will remain anonymous all throughout the end of the study.
