RELEASE AND QUITCLAIM

STARWORKS INTEGRATED SERVICES INC. (SWIS INC.)

organized and existing in accordance with the laws of the Philippines, having offices at
Unit 107 Parc Royale Condominium Jade Drive, Ortigas Center, Pasig City and

WHEREAS, I have terminated my employment with STARWORKS

INTEGRATED SERVICES INC. and
_________________________________________and I hereby desire to release and
forever discharge the said companies and its officers from any and all manners of action,
debts, dues contracts, judgements, damages and all claims by way of wages, separation
pay, overtime pay, differential pay, incentive leave pay, holiday pay or otherwise as
maybe due me incident to my past employment with said establishments.

NOW THEREFORE, for and in considerations of the sum of ________________

receipt of which is hereby acknowledged and confessed, I do hereby release and forever
discharge said STARWORKS INTEGRATED SERVICES INC., and

separation pay, overtime pay, differential pay, incentive leave pay, holiday pay or
otherwise as may due me incident to may employment with said establishments. I here by
state further that I have no more claim, right or action of whatsoever nature whether past,
present or contingent against the said companies and/ or its officers.
