REFERENCES

v Should make use of the current APA Format (7th edition). (Refer to APA
Referencing). For both in-text citations and reference lists.
v Arrange all references in alphabetical order.

APA FORMAT

In-text citations

In APA Style, cite your sources by putting the information about the source in parentheses
at the end of a sentence or in the text of your paper as opposed to a footnote where the
source information is at the bottom of the page or an endnote where it goes at the end of
your paper. There are slight differences depending on which style you are using.

v Give the author’s last name and the publication year.


v Only use page numbers or paragraph numbers for a direct quote.
v Make sure the source information in parentheses matches with your reference in
the reference list. The punctuation for the sentence goes AFTER the parentheses.
v For a quote less than forty words put quotation marks around the quoted words. For
sources with designated page numbers - if the author and date are introduced in the
sentence as a narrative citation, then add the page number in parentheses at the end
of the quote. If the source does not have designated page numbers, then add the
paragraph number, heading, or a combination of both the heading and paragraph
number. If the author and date are not introduced as part of the text, then include
the author and date with the page or paragraph number. The period should come
after the parentheses.
v If your quote is more than forty words, set it off in a block text by beginning the
block quote on a new line, indent 0.5 inches (one-half), and do not add quotation
marks around the block quote. At the end of the quote put the period after the last
word of the sentence followed by the parentheses. For more information, see Block
Quote.

Some More Basic Reference Page Rules

v Your references should be alphabetized by the last names of the first author of each
source.
v All references should be double-spaced. But in the university it is single-spaced.
v Each reference should use a hanging indentation: the first line of the reference
should be flush left, but each additional line of the reference needs to be indented.
v In article titles, only the first letter should be capitalized. If a colon appears in the
title, the first letter after the colon should also be capitalized. The title should not
be placed in quotations, underlined or italicized.
v All major words in the title of a journal should be capitalized; i.e. The Journal of
Personality and Social Psychology.
v Longer works such as books and journals should appear in italics.

INFORMED CONSENT FORM

I understand that I am being asked to participate in a research study. This research study
will explore the lived experiences of Cebuano healthcare frontliners on their experiences
on the COVID-19 pandemic.

If I agree to participate in the study. I will be interviewed for approximately 45 – 60 minutes


about my experience on the COVID-19 pandemic as a healthcare frontliner. The interview
will take place in a private area and most convenient to me. No identifying information
will be included after the data gathering. I understand I will not receive any monetary
consideration for participating in the study. I do not anticipate any risks from participating
in this research study. My participation in the study will not cause physical nor
psychological harm. The study will be beneficial to the respondents and the community
because the findings of this study will ensure implications to groups of individuals who
will benefit from the study.

I realize that the knowledge gained from this study may help either me and all stakeholders
to know more and understand about the experiences of the healthcare frontliners on the
COVID-19 pandemic.

I realize that my participation in this study is entirely voluntary, and I may withdraw from
the study at any time I wish. If I decide to discontinue my participation in this study, I will
continue to be treated in the usual and customary fashion.

I understand that all the data will be kept confidential. However, this information may be
used in publication or presentations.

I understand that in the event that any research related activities result to cause harm. I will
not be automatically compensated by the hospital. If I have any questions or concerns
