In preparation for the Mandatory/Random Drug Testing Program for the 2nd Semester 2022-2023
for 1st and 3rd-year students for all schools/colleges and 2nd year HRM students for NHM, we are sending
you the forms of Notices for Drug Testing (NDT) and Consent Forms for distribution to your students.

Procedures for the distribution and collection of the forms are as follows:
1. The online Notices for Drug Testing (NDT) and Consent forms shall be shared to all enrolled 1 st and
3rd-year students and 2nd year HRM students by their respective Schools.
2. The Deans shall inform students to print out completed and signed forms and bring them
individually during the drug testing schedule to be submitted to the CEU Clinical Laboratory.
3. Students should be reminded to bring their COM’s for verification purposes.
