Subject: Fire Incident Report at Work Place

Dear Sir,

In Negros Occidental Electric Cooperative (NOCECO) San Enrique Sub -Station,

situated at Rizal St., Brgy. Poblacion, San Enrique, Negros Occidental, the primary line and
transformer was razed by fire that caused the explosion from the main conductor and
transformer with undetermined cost of damages, during the fire incident that transpired
at aforementioned place at about 6:20 in the evening of April 15, 2022.

Dear Sir:

The primary line of NOCECO was cut off, touching the secondary and neutral line that
caused fire which damaged the grocery store owned by Mr.Eusebio Quitco Y. Retesa Jr
63 years old, married, tricycle driver of Prk.Santan, Brgy.Tibsoc, San Enrique,Negros
Occidental. The said indemnities includes grocery items, mobile phones and appliances
(televisions and refrigerator).

The incedent happened at 6:00 O’clock in the evening of April 15,2022.

The primary line of NOCECO was accidentally cut out and the electrical wire fell down on
the roof house of Ms.Jan Marie Jolipa y Eboseo,24 years old,single and resident of Sitio
Cadulunan, Brgy Miranda, Pontevedra, Negros Occidental.

The incident happened at about 11:30 pm of April 20,2022 that caused damages the
appliances and other electrical items of the said owner.

Herewith the attached proof of indemnities.

Dear Sir:

The Feeder 2 San Enrique Sub-Station primary line conductor was cut off simultaneously
last April 15, 2022 at 6:20 in the evening, from San Enrique Public Cementary to Prk.
Lourdes San Juan, Pontevedra . The third shift on duty Mr. Sarex Alonso and Mr. Antonino
Garcenilla together with BOLOVA Crew Mr. Aron Pura and June Paulo Dellero noticed that
the pole in Feeder 2 burning same with transformer 10MVA. The BOLOVA Crew hurriedly
turn off the control panel but failed to operate because of the line circuit is melting.

The fire trucks arrived 20 minutes after the fire started, and within 30 minutes they
manage to clear the area and it was declared fire out. Mr. Aron Pura immediately
informed the Radio Room operator Mr. Carlo Panogadia, Main Kabankalan regarding the
incident in San Enrique Sub-Station.

In relations to this happening, affected consumers on that area complaining about the
damaged to their appliances due to high voltage situation.
