    on December 3 and 4 , 2022 at NAGA CENTRAL ELEMENTARY SCHOOL, Naga, Zamboanga Sibugay .

    I have considered the benefits that my child will obtain from his participation provided that
    due care and precautions will be observed to ensure the comfort and safety of my child and that DepEd
    employees and personnel may not be held responsible for any untoward incident that may happen
    beyond their control.
