PARENTAL CONSENT

I/We hereby willingly and voluntarily give consent for the participation of

my/our/son/daughter _____________________________________ to join the 2023 Regional Schools

Press Conference (RSPC) as contestant this coming June 17-18, 2023 at Bislig City.

In connection to this, I/We will allow my/our/son/daughter to attend the said event. I have

considered the benefits that my/our/son/daughter/will derive from his/her participation in this activity

provided that due care and precaution will be observed to ensure the comfort and safety of my

son/daughter and that DepEd employees and personnel may not be held responsible for any untoward

incident that may happen beyond control.
