Exhibit “1” – Job Order from ISLI printed on October 4, 2022 –
to prove that there was an order to inspect and disconnect Account
No. P6969 because the account was disconnected last March 15, 2022
due to delinquency but was reported lighted on site.

Exhibit “2”- Request Letter for Police Assistance dated October

4, 2022 - to prove that at the time of the incident, the ISLI team
requested for police assistance during their operation to disconnect
Account No. P6969.

Exhibit “3” – Inspection Report with Notice of Violation and

Disconnection – to prove the violations of the accused in connection
with Account No. P0199.

Exhibit “4” – Police Extract Blotter issued to John Mayer – to

prove that the incident regarding the violation of RA 7832 – Anti-
electricity Pilferage Law was reported for recording in the Police
Extract Blotter Book.

Exhibit “5” – Incident Record Form – to prove that the victim

reported the injuries he sustained from the accused last October 4,
2022.

Exhibit “6” – Police Extract Blotter issued to Joe Alwyn – to

prove that the incident regarding the injuries he sustained from the
accused was reported for recording in the Police Extract Blotter Book.

Exhibit “7” – Medico Legal Request – to prove that the PNP

requested for assistance to have the victim checked.

Exhibit “8” – Medical Certificate of Joe Alwyn – to prove that

the complaining witness sustained a contusion on his left lower jaw.

Exhibit “9” – Certificate to File Action – to prove that the case

was referred previously to the Barangay before filing the complaint.

Exhibit “10” – Judicial Affidavit of Witness – John Mayer

Exhibit “11” – Judicial Affidavit of Witness – Taylor Lautner

IV. WITNESSES TO BE PRESENTED

1. Joe Alwyn – the victim and the complaining witness, to

prove the facts surrounding the incident where he sustained
injuries from the accused.

2. John Mayer – staff engineer of ISLI, to prove that he was one

of the members of the team which conducted the operations
last October 4, 2022 concerning the account P6969.
3. Taylor Lautner – Lineworker-Revenue Protection Group of
the Techinal Services Section of ISLI – to corroborate the
testimony of Alwyn and Mayer.
4. Joe Jonas – Lineworker-Metering – to prove that he was one
of the members of the operation, and that he personally saw
the tampered meter of Account P6969.
5. PSSG Katy Perry – officer who assisted John Mayer and Joe
Alwyn in their police extract blotter and incident record
form.
6. Dr. Selena Gomez – attending physician of the victim, to
prove the injuries sustained by the victim as reflected in the
Medical Certificate.

V. TRIAL DATES
