 Obtain all work permit –

 HOT WORK PERMIT
 EXCAVATION PERMIT
 LIFTING PERMIT

 Ensure supervisor brief the crane operator

about the job before start work everyday
1.1 Unloading tank plate from lorry and store at -falling tank plate from  The entire unloading area should have
beside every of tank foundation by using crane crane unauthorized entry
-Hit by lifting tank plate  Ensure all rigging materials and lifting hook
in good condition
-toppling of crane  Crane should be placed on a stable and even
ground surface with outrigger cribs and fully
extended.

FABRICATION: -Hit by tank plate  Ensure tank plate been rolling by expertise
2.0 Tank Plate rolling and fabrication person.
 Ensure rolling work area been barricaded
-Flying fire heat spark  Ensure workers use face shield and glove
-fire  No flammable materials should be keep at
fabrication site
-Electrocution  Ensure welding machine and cable in good
condition.

INSTALLATION OF TANK PLATE

SHELL COURSE:
3.0 Install tank base plate by using crane - falling tank plate from  Ensure all rigging materials and lifting hook
crane in good condition
-Hit by lifting tank plate
-toppling of crane  Crane should be placed on a stable and even
ground surface with outrigger cribs and fully
extended.
3.1 Install tank plate shell course by using crane - falling tank plate shell
course from crane
-toppling of crane

TANK ERECTION and MODIFICATION
WORKS: -Flying Heat Spark
4.0 Welding installed tank plate shell course -Fire
-Electrocution

4.1 -Cuts to workers hand

-Explosion
4.1 Install tank roofing by using crane - falling tank roofing plate
from crane
-Hit by lifting tank roofing
 Ensure all rigging materials and lifting hook
in good condition
 Crane should be placed on a stable and even
ground surface with outrigger cribs and fully
extended.
 Ensure tank plate shell positioning correctly
and tank support tools should be fixed at tank
plate shell course
 Ensure tag line been used during lifting and
installing tank plate shell course
 Ensure welding work area been barricaded
with signage.
 Ensure fire Extinguisher kept at tank erection
site.
 Ensure genset and welding machine in good
condition
 Ensure workers using glove during grinding
works
 Workers should wear face shield
 Ensure hoses in good condition and soapy
had been carried out weekly.
 All oxyacetylene cylinders should have
flashback arrestor.
 Ensure all rigging materials and lifting hook
in good condition.
 Ensure tank roofing plate been secured well
 plate in rigging equipment.

extended.
 Ensure load chart and SWL of crane
followed.

4.2 Painting and modification works -Falling materials  Ensure work are been barricaded with proper
-spillage of paint signage
 Ensure paint been store at specific storage
area.
