SECOND LEVEL ASSESSMENT


Guide and Sample Interview Questions

POOR ENVIRONMENTAL CONDITION


I. Determine if the family recognizes the existence of the condition or problem. If the
family does not recognize the presence of the condition or problem, explore the reason why.

a. What do you think about your home environment condition ? do you know any factors
that may cause infection or problem in your home?(Ano ang palagay ninyo sa kalagayan ninyo?)
Answer: “Ayos naman. Okay naman as long as ma-maintain ang kalinisan“

b. How about the cultured mosquito larvae that you feed your fishes? What can you say
about that?
Answer: “okay lang as long as yung mga kiti-kita ay nauubos, habang nasa larvae stage sya
kailangan mapaubos na ito agad”

III. If the family recognizes the presence of the condition or problem, determine if
something has been done to maintain a wellness state or resolve the problem. If the family
has not done anything about it, determine the reasons why. If the family has done
something about the problem or condition, determine if the solution is effective.

a. What have you done to improve the condition or situation? Or to maintain your safety
while doing that?
Answer: “Safe naman dahil yung ginagawa naming paglalagay ng tubig sa container medyo
malayo sa bahay. Parang nagiging trap para sa mga lamok dahil yung tubig na nilalagay
naminpag naka-itlog na doon yung lamo, sigurado yung mga kiti-kiti na magiging anak ay
ipapakain naming sa mga isda.”
b. What are your plans regarding this? (Ano ang inyong binabalak tungkol dito?)
Answer: “Panatilihin lang yung regular na pag-monitor ng iniitlugan ng mga lamok para ma-
check ito agad.”

c. What improvements in the condition of …have been observed? (Anong mga pagbabago
ang inyong napansin sa kalagayan ni …?)
Answer: “So far wala naming nangyayaring masama “

IV. Determine if the family encounters other problems in implementing the


interventions for the wellness state/potential, health threat, health deficit or crisis. What
are these problems?

a. What were the problems or barriers encountered in…?


(anu-ano and inyong nagging problema sa pagpapatupad ng mga solusyon sa ang inyong nagging
problema sa pagpapatupad ng mga solusyon sa…?)
(anu-ano ang mga nagging sagabal o balakid nang inyong ginawa ang..,?)
Answer: “Wala naman”
b. Do you wear protective gears or anything that can protect you from possible health threat
by doing that?
Answer: “Wala, mahalaga naman ay ingat lang at pag-aralan ng ayos paano mag-cu;ture ng mga
kiti-kiti.”

c. Why are you not considering using protective gears…?


(Ano kaya ang dahilan kung bakit ninyo itinigil o hindi pinagpatuloy and dati ninyong ginagawa
Answer: “Sanay na rin naman ako at wala din naming kompletong kagamitan para doon.”

V. Determine how all the other members are affected by the wellness state/potential,
health threat, health deficit or stress point.

a. How are the other members affected by…?


(Ano ang nagging epekto ng…sa ibang miyembro ng pamilya?)
Answer: “Wala naman”
b. How are the other members reacting to…?
(Ano ang nagging reaksyon ng ibang miyembro ng pamilya sa…
Answer: “Ayun, as of now wala naman silang complain. Basta lang daw panatilihin yung pag-
monitor”

Problem I. Poor Environmental condition


1. Inability to provide a home environment conducive to health
maintenance and personal development due to inadequate knowledge of preventive measures.
“Wala, mahalaga naman ay ingat lang at pag-aralan ng ayos paano mag-culture ng mga kiti-
kiti.”
2. . Inability to recognize the presence of the condition or problem due to inadequate
knowledge.
““okay lang as long as yung mga kiti-kita ay nauubos, habang nasa larvae stage sya kailangan
mapaubos na ito agad””

Mr. Jay Barrientos stated that he sees no problem regarding the way he feed his fishes by
culturing mosquito larvae as long as he keeps an eye on it and feed them to the fishes on time.
He also added that they keep the container of larvaes far enough from the house and thinks that it
somehow helps in controlling the pests of mosquito larvae’s even before it matures into
mosquitoes. His family are not bothered by it as long as no one got sick because of culturing
mosquito larvae.
HYPERTENSION

I. Determine if the family recognizes the existence of the condition or problem. If the
family does not recognize the presence of the condition or problem, explore the reason why.

a. What do you think about your mom’s condition ? Is she aware of it (Ano ang palagay
ninyo sa kalagayan ninyo?)
Answer: “I think Okay naman si mama kasi may iniinom na maintenance at Aware naman sya,
alam nya ang mga bawal at hindi”

II. If the family recognizes the presence of the condition or problem, determine if
something has been done to maintain a wellness state or resolve the problem. If the family
has not done anything about it, determine the reasons why. If the family has done
something about the problem or condition, determine if the solution is effective.

a. What have you done to improve the condition of your mom?


Answer: “malaking impact yung pagkain.. pinapayuhan naming si mama palagi. Yun lang din
naman ang alam kong pwedeng gawin kasi may maintenance narin syang iniinom ”
b. What are your plans regarding this? (Ano ang inyong binabalak tungkol dito?)
Answer: “ Palagi lang naming sya pinapaalalahanan kasi minsan nakakalimot sya. Hindi nya
namamalayan na mapupunta na sya sa high blood”

c. What improvements in the condition of …have been observed? (Anong mga pagbabago
ang inyong napansin sa kalagayan ni …?)
Answer: “Sa tingin ko ngayon, since nagtagal ako doon nung pandemic dahil nga pinapalalahan
ko rin sya palagi kaya naiiwasan nya ang mga bawal“

IV. Determine if the family encounters other problems in implementing the


interventions for the wellness state/potential, health threat, health deficit or crisis. What
are these problems?
a. What were the problems or barriers encountered in…?
Answer: “Di naming ma-monitor regularly ang blood pressure ni mama dahil wala kaming BP
apparatus,Yun nga yung di nya din makontrol ang kanyang sarili, minsan nagagalit o di kaya
nakakakain ng bawal”

health threat, health deficit or stress point.

a. How are the other members affected by…?


(Ano ang nagging epekto ng…sa ibang miyembro ng pamilya?)
Answer: “Dahil nga sya yung nanay, sy anag-aasikaso lahat. Kapag maysakit si mama sympre
kaming mga anak ang gumagawa ng responsibilities nya. Malaking kawalan pag si mama ay
maysakit. Si papa ay umaabsent pa sa trabaho minsan kapag maysakit si mama”

b. How are the other members reacting to…?


(Ano ang nagging reaksyon ng ibang miyembro ng pamilya sa…
Answer: “Para sakin kailangan ma-maintain ni mama yung kanyang kalagayan na maayos, kaya
ngs sya may gamut para di lumala ang kanyang sakit. Si papa ang isa din nagpapaalala palagi
kay mama. Malungkot din kami sa kalagayan nya kasi nakakapg-alala kaya palagi na
pinagdadasal na maging normal na yung blood pressure nya.

Problem 2
1. Inability to provide adequate nursing care to the sick/ at risk member of the family due
to Lack of the necessary equipment and supplies of care.
“Di namin ma-monitor regularly ang blood pressure ni mama dahil wala kaming BP
apparatus,Yun nga yung di nya din makontrol ang kanyang sarili, minsan nagagalit o di kaya
nakakakain ng bawal”
2. Inability to provide adequate nursing care to the sick/ at risk member of the family due
to inadequate knowledge about the health condition (nature,
severity, complications, prognosis and management)
“I think Okay naman si mama kasi may iniinom na maintenance at Aware naman sya, alam nya
ang mga bawal at hindi”
Jay stated that although his mother is aware of her condition, she still needs to be reminded all
the time. He also stated that apart from the reminders, he doesn’t know what else to do to help
his mom. They do not have a BP apparatus to monitor the blood pressure regularly hence hey
weren’t able to monitor her condition. The rest of the family is worried about their mom, and it
affects their daily lives because it is their mom who does most of the chores and supervises them
what to do.
