    1. Copy/scan full page of Passport


    2. 12 months VISA
    3. Original Residence Permit (e-KITAS) for full one year (12 months), Any less than
    12 months, a tax and duty are applicable
    4. Original Work Permit (IMTA) for full one year (12 months), Any less than 12
    months, a tax and duty are applicable.
    5. Exit Permit Only (EPO)
    6. Flight Ticket
    7. PP 8 / PP 19 Form
