AFFIDAVIT OF DESTRUCTION OR MUTILATION

I, SHEENA LIZA TORRES YANONG, Filipino, of legal age, married, with residence
at Purok 1, Raw-an, Lala, Lanao del Norte after having been sworn to in accordance with
law, hereby depose and state:

1. That I am the owner of Philippine passport number EB8735746 which was

issued by DFA Cagayan de Oro on 22 July 2013 and valid until 21 July 2018;

2. That the said passport was destroyed or mutilated because I unknowingly placed
it in my bag and there was moisture accumulated in the storage cabinet;

3. That the damage on the said passport has rendered it unusable for travel
purposes;

4. That I am executing this Affidavit to attest to the truth of the foregoing facts and
to explain the circumstances how my passport was destroyed or mutilated;

5. That I am further executing this Affidavit to support my application for a

replacement of my Philippine passport at the Department of Foreign Affairs and for
whatever legal purpose it may serve.
