AFFIDAVIT OF LOSS

I, Duke Trump, of legal age, Filipino and with residence address at_________________, Gen.
Trias City Cavite, Philippines, after having been sworn in accordance with law, do hereby depose and
say:

1. That I am a duly-licensed driver in accordance with Land Transportation laws, rules and
regulations and was issued a corresponding Professional Driver's License with No. xxxxxxx and
expiring on June 3, 2028 and issued at Land Transportation Office (LTO) Imus City Cavite;

2. That sometime of June 5, 2023 while partying together with my colleagues, I accidentally loss
my wallet together with my aforesaid license;

3. That despite diligent effort to locate the same it was nowhere to be found and is now beyond
recovery;

4. That the Driver's License has not been confiscated, red tagged nor any pending cases to Land
Transportation Office or to any transportation authority for any traffic violations;

5. That I am executing this affidavit of loss to attest to the truth of the foregoing and to support my
application for the issuance of new driver's license to Land Transportation Office.
