HOUSE RULES THERAPY UNDER PRIVATE:

1. Please attend to your schedule on time or at least 5 minutes earlier before.

2. Fetchers/Guardians are advised to fetch on time or at least 5 minutes earlier before to
safeguard security and welfare of their wards.
3. For changes or cancellations, kindly inform us at least a day before your schedule.
4. Early cancellation of schedule would not lead to penalty instead you are required to have a
make up session to cover up your absences. No make up session would automatically lead to
penalty fee of Php 400.00.
5. Absences without prior notice would automatically lead to a penalty fee of Php 400.00.
6. Notification of cancellations or changes in schedule is from 8 am to 5 pm only (during office
hours only). After 5 pm of cancellation would lead to penalty fee of Php 400.00.

HOUSE RULES THERAPY UNDER FINANCIAL ASSISTANCE:

1. Once you received your child’s Financial Assistance, the certain amount released by the DSWD
will be turned over to SL Therapy Center. (Therapist fixed rate).
2. The center will give you the schedule and covered dates based on the amount released by the
DSWD.
3. Attend to your schedule based on the covered dates given to you by the center.
4. Please be reminded that there will be no penalty and make up sessions. Covered date session
on that day would automatically forfeited.
Therapist will give take home activities to cover up your child’s absences.

RULES ON PARENT-THERAPIST CONFERENCE (IEP and PROGRESS REPORT)

1. The center will have to schedule your conference with the therapist regarding your child’s IEP
and progress report on the same schedule (same stated date and time schedule) of your child.
2. Parent conference will be given on the same day of therapy (last 30 minutes of the therapy
session).
