• Please read all information thoroughly and note that all fields are required for the site survey to be considered
complete.
• Please note that Avida is not responsible for setting up base builds at the site. If this is requested, please refuse,
make note on this form, and have the Dealer follow up with Avida.
• Please submit all photos with this survey form as per the Photo Checklist on page 6.

1. Site Survey Package from Avida (including but not limited to: floor plans, architectural plans, site photos, site
order information)

CODE OF CONDUCT FOR SURVEY REPRESENTATIVES:

1. Neat attire is required
2. Always check in with site contact upon arrival, prior to commencing the inspection
3. Respect any dealership security concerns
4. Obtain approval from on-site contact prior to taking photos
