Respectfully submitted herewith is the Daily Activities Report covering the period from 0700H 07 to 0700H 08
of December 2022. For your information and ready reference.

I. INCIDENT:

1. At around 1305H dated 07 December 2022, the Orient Square building have experienced unusual movement
of the building, which lasted for about just few seconds only. And no reading on the seismic accelerograph
device located at 6th floor Engineering Office. That according to the record from PHIVOLCS ( Philippine
Institute of Volcanology and Seismology ), the recorded intensity was intensity II at Pasig City. So, most of the
tenants have not felt the Earthquake. But some tenants voluntarily evacuated from their respective units using
fire exit going to ground floor and to the evacuation area in front of the building at Don Francisco Ortigas, Jr.
Road side. Assisted by Orient Square Emergency Response Team, Headed by the Incident Commander, Ms.
Sherdy C. Grabanzor. That the elevators were temporarily parked at ground floor for safety. Also, the parking
Entrance & Exit and the back door Entrance/Exit at Ruby Road side was temporarily closed. That the voluntary
evacuation started at around 1322H. Then after the last man Sg. Jay Satago, cleared the area from 37 th floor going
down and arrived at ground floor at around 1347H. After 3 minutes, at around 1350H the Incident Commander,
Ms. Grabanzor, announced that those who evacuated can go back to their respective units or Offices. That no
report of anyone got hurt due to this incident. Also, no damage of building equipment & facilities, and building
structure.

II. SIGNIFICANT EVENT:

1. At around 0850H to 1715H dated 07 December 2022, two (2) personnel of Twincom/Globe, Mr. JD Dalisaymo
and Mr. Jomar Pelagio, conducted taking pictures of antenna, service unit component at 25 th floor unit-2503,
Iljin Wall Tech, Inc., 30th floor unit-3001 Golden Suntec Solutions, Inc., and at 35 th floor Blast Asia, Inc.,
monitored by Roving guard, Jay Satago.

2. At around 0850H
to 1000H dated 07 December 2022, Engineering Technicians, Mr. Nick Dela Cruz, and Mr. Chris Villadiego,
conducted dismantling of Transfer pump motor no. 2, at basement 6 cistern tank room, due to unusual noise on
motor pump during operation. Monitored by Basement 6 Roving guard, Roberto Mamaril.

3. At around 1205H to 1328H dated 07 December 2022, DPI-PAG. & PMG. Officers, Engr. Condrad V. Roque,
Engr. Vince Yap, Engr. Mark L. Alulod, Engr. Jaydick R. Leope, with TOSCAI-PMO, Ms. Sherdy C. Grabanzor,
Engr. Josenily J. Nautan, Jr., Engr. Marven Oropel, & Colliers Senior Manager, Mr. Dennis Marquez, had a
meeting at 6th floor Engineering Office with Kone Modernization Project Manager, Mr. Charles Reyes, regarding
neutral tapping. Also, the group, conducted inspection inside the Substation room at 6 th floor. Monitored by
Roving guard, Jay Satago.

4. Continuation: At around 2012H dated 07 December 2022, to 0045H 08 December 2022, four (4) personnel of
Kone Installation Team, Mr. Rudy Paralejas, Mr. Jeffrey Arada, Mr. Honieval Olabre, and Mr. Redencio
Timblaco, conducted installation of plastic green mesh screen in between highrise elevator no. 2 & 3, from 37 th
floor to ground floor inside the tunnel. Assisted by three (3) personnel of Kone Modernization Team, Mr.
Gregorio Cruz, Mr. Godofredo Lining, and Mr. Ryan Dacuno. Monitored by Roving guard, Felipe Layan.

5. At around 2030H to 2050H dated 07 December 2022, Engineering Technicians, Mr. Dennis Bernal and Mr.
Daniel De Vera, conducted checking and functionality testing of the Earthquake device at ground floor electrical
room, 18th floor and 37th floor . And the result is normal. The device is functioning. Monitored by Security
Officer, Carlo Belleza.
