    I am writing to request your assistance in updating my account name on LERIS.


    Unfortunately, last April 3, 2023, I requested to retrieve my email using my Spouse's Surname which
    is a mistaken, because I have not changed my status in PRC yet.

    I understand it is my fault and your company has strict privacy policies and I am willing to
    provide any necessary information to verify my identity to access my email account. I have attached
    copy of my government-issued ID to this letter to help with the verification process.

    I would appreciate it if you could update my account name on LERIS as soon as possible and
    provide me with the necessary steps to access my account. This is an important step to renew and
    update my PRC License.
