The Accountancy and Accounting Information System would like to express its gratitude and
appreciation to you and your office in accommodating our students on their on-the-job training
(OJT)/Internship program this semester. As part of our commitment to provide a high-quality
learning experience for our students, it is essential for us to monitor their progress and ensure that
they are acquiring the necessary skills and knowledge and instill values in your workplace. With this,
we are writing this request for your valuable assistance in conducting the initial evaluation on the
performance of our OJT interns deployed in your esteemed institution.

We highly appreciate your ongoing support in accommodating our OJT interns and understand the
importance of maintaining effective communication during the internship period. Your initial
evaluation and feedback will be used to assess the interns' strengths and weaknesses and will allow
us to gauge the performance and development of our interns and identify any areas where they may
need additional support and guidance during the first half of their assignment to ensure that they
are making the most of their OJT experience. Further, this assessment/information will help us to
develop an adapted training plan for each intern, if applicable.

Attached here is a copy of the evaluation form. May we request your assistance in completing the
form for each intern within a week upon receipt to give us sufficient time to review the feedback and
discuss any necessary adjustments or improvements with our interns. Kindly have it sealed for
confidentiality purposes.
