AFFIDAVIT

I, ANGELINA B. SUAREZ, Filipino, of legal age, widow, and a resident of

Guripan, Mahayag, Zamboanga del Sur, Philippines, after having been duly sworn to in
accordance with law, hereby depose and state, THAT:

1. Sometime in May 1963, I and MANOGANGCAR GUNTING, have been

cohabiting as husband and wife without the benefit of marriage;

2. That during our cohabitation, I am still a minor child, being at the age of 13
years old, while Manogangcar Gunting, a maranao, is 35 years old;

3. On March 15, 1967, my live-in partner, MANOGANGCAR GUNTING died

at Lumbay, Nagi, Lanao del Sur. He was found dead at our farm with full of
stabbed all over his body and some wounds that was probably hit by a gun;

4. That I am executing this affidavit to prove the veracity of the facts mentioned
herein and to support for my claims for the death benefits of my second and
legal husband, the late EUGENIO D. SUAREZ, JR., from the Social Security
System (SSS) and for whatever legal purpose this affidavit may serve best.

IN WITNESS WHEREOF, I have hereunto set my hand this 24th day of July
2015, in Molave, Zamboanga del Sur, Philippines.
