AFFIDAVIT

I, VIRGINIA C. DEJITO, of legal age, Filipino, married, and a

resident of Mabini Street Corner Quezon Extension, Barangay Maloloy-
on, Molave, Zamboanga del Sur, after having been sworn to in
accordance with law, hereby depose and say:

1. That I am a live-in partner of the late Julie Sabela, who died in

Molave, Zamboanga del Sur on April 16, 2011;
2. That we have been living in the concept of husband and wife
without the benefit of marriage since 1997 or for a period of more
than 14 years;
3. That out of our relationship was born Karen Caye Dejito Sabela and
Aliah Mae Dejito Sabela;
4. That I am executing this affidavit to attest to the truthfulness of the
foregoing facts and to support for my claims for the death benefits
of the late Julie Sabela from the Social Security System and for
whatever legal purpose this affidavit may serve best.

IN WITNESS WHEREOF, I have hereunto set my hand this 2 nd day
