AFFIDAVIT OF UNDERTAKING

I, LEONILA MEJOS TANALEON, of legal age, Filipino, and a resident

of Lupon, Davao Oriental after being duly sworn to, under oath and in accordance
with law, do hereby depose and state that:

1. I am married to a certain BERNABE TANALEON of Isulan, Sultan

Kudarat;

2. That my estranged husband has already abandoned me and our kids

and was never able to exert effort to communicate with us since 2001;

3. My husband has gone missing since 2001, or for almost 21 years now;

4. I have exerted all efforts to locate and communicate my estranged

husband but still to no avail;

5. The said abandonment affected me and my family deeply, that I have to

be the father and the mother at the same time to my kids and brought
burden to us all along;

6. I undertake to file a petition in court for presumptive death in due time;

7. I undertake that all the abovementioned are truthful and was not made
up and this undertaking is executed for whatever legal purpose this may
serve.
