AFFIDAVIT OF ABANDONMENT

I, ANNA MELINDA VILLANUEVA y LEVISTE, Filipino, of legal

age, married to Paolo Joseph Villanueva y Santos, with residential
address at Unit 901 Osmeña west Bldg., Don Antonio,
Commonwealth, Quezon City, Metro Manila, after having sworn in
accordance with law, do hereby depose and state:

1. That I, ANNA MELINDA VILLANUEVA y LEVISTE and

GLENE DIOR ABALOS y GONZALES were common law
spouses from 2004 up to 2007;

2. That our relationship was blessed with one (1) child, namely:

A. STEPHEN MICHAEL GLENE ABALOS y LEVISTE,

born on 5 June 2005, a minor.

3. That our relationship turned sour and it yielded a break up

resulting to the loss of communication between me and Mr.
GLENE DIOR ABALOS y GONZALES on 2007, that was also
the last time I saw and spoke with the father of my child,
STEPHEN MICHAEL GLENE ABALOS y LEVISTE;

4. That since 2007 up to present date Mr. GLENE DIOR ABALOS

y GONZALES abandoned me, ANNA MELINDA VILLANUEVA
y LEVISTE and his child, STEPHEN MICHAEL GLENE
ABALOS y LEVISTE, and his whereabouts are unknown to us.

5. That I, as the mother of STEPHEN MICHAEL GLENE

ABALOS y LEVISTE, is exercising/providing solo parental
authority, care, and support to our child since 2007;

6. That I considered the act of Mr. GLENE DIOR ABALOS y

GONZALES, forsaking me and his child as an act of
abandonment;

7. That I had met Mr. Paolo Joseph Villanueva y Santos on

2010 and had a relationship with him on 2011.

8. That I was subsequently married to Mr. Paolo Joseph

Villanueva y Santos, on 25 October 2017 at Manila City Hall;

9. That since the wedding up to present date, my husband and I,

Mr. Paolo Joseph Villanueva y Santos are exercising joint
 parental authority de facto over the child, STEPHEN MICHAEL
GLENE ABALOS y LEVISTE, and providing for his care and
support;

10. That I am a solo parent to minor child, STEPHEN

MICHAEL GLENE ABALOS y LEVISTE who took all the
burden of raising, rearing, and supporting said child from 2007
up to 2017;

11. That I am executing this affidavit to attest to the truth of

the foregoing facts, particularly that of the abandonment of Mr.
GLENE DIOR ABALOS y GONZALES to minor STEPHEN
MICHAEL GLENE ABALOS y LEVISTE, and for whatever
legal purpose this affidavit may deem serve.
