AFFIDAVIT OF SUPPORT
I, SANNY DARACIN LUNA, of legal age, single, Filipino, and a resident of
Bulacao, Talisay City, Cebu, after having been duly sworn to in accordance with
law, do hereby depose and say:

1. That I am the brother of the late Mary Ann Darasin Luna a bona fide
member of the Social Security System (SSS);

2. That she died on February 7, 2017 at Perpetual Soccour Hospital,

Cebu City;

3. That she has two minor children namely: Mark Danie Luna Gonzales
who is eleven years old and Molly Deniece Luna Gonzales who is
three years old;

4. That upon my sister’s demise, I took over the care, custody and
support of Molly Deniece, who is living with me in my house in
Bulacao, Talisay City;

5. That I am now processing the application for the pension of the

children of my sister and one of the requirements is the Affidavit of
Support;

6. That I execute this Affidavit in order to attest to the veracity of the

foregoing statements, to support my application for the pension of the
children of my sister and for whatever legal purpose this may serve
best and useful.

IN WITNESS WHEREOF, I have hereunto set my hands this 9 th day of
