Dear Sir & Madam,

I, Divina G. Nuñez the wife of the deceased member Severino R.


Nuñez is writing to you this letter to make an appeal regarding my
application for Survivor Benefits Plan (SBP) and/or Retired
Serviceman Family Protection Plan (RSFPP).

It’s been said from the letter that I received from the Defense
Finance Accounting Service, by MR. J. Stewart ( Military Pay
Technician Retired and Annuity) that on the DFAS office the record
only reflect child coverage only elected by my late husband Severino
R. Nuñez for our children’s only, Marjorie G. Nuñez and Michael G.
Nuñez.

Now, Marjorie G. Nuñez claimed all her Survivor Benefit Plan


(SBP) but my son Michael G. Nuñez have not received or claim and
Survivor Benefit Plan (SBP) benefits, since 1994, and from memory he
was only nine (9) years of age back then, I would like to ask if Michael
G. Nuñez could claim the benefits that he did not received from the
said date Spetember 1994.

In regards with this appeal as well, I would like the Department


of Defense Finance and Accounting Service to know that I Divina G.
Nuñez have not received any benefits from the Veteran’s Affair
Philippines (VA) File No. XC 29 262 152, Severino R. Nuñez.

Also, I went to Retired Activities Office (RAO) in Angeles,


Pampanga Philippines, they have told me that my Veteran’s Affairs
cheque is for life. Retired Activities Office (RAO) Philippines then
called the Veteran’s Affair Philippines (VA) and they told me that they
cancelled my benefits because I am living with someone. I want to
clarify if this reason of Veteran’s Affair Philippines (VA) gave me is
under the DFAS law? If the Veteran’s Affair Philippines (VA) reason on
why they cancelled my benefits is true, can the Veteran’s Affair
Philippines (VA) do an investigation on my records from year 1994 up
to now? In regards also with the monthly cheque’s, that I never
received under my name Divina G. Nuñez.

At the moment, I am 53 years of age, not married and does not


have any income or benefits to use in my years to come.

I am hoping that the DFAS can give me a chance to give my


records another look as I am in poor condition and trying to survive.
