AFFIDAVIT

I, MARY GRACE BUENAVISTA ANSELMO, Filipino, of legal age, married to

ANTHONY TAPANG ANSELMO and a resident of Purok 6, Sinasajan, Peñaranda, Nueva
Ecija, Philippines after having sworn to in accordance with law depose and state;

1. That, I am the wife of ANTHONY TAPANG ANSELMO (Anthony for brevity).

Attached herewith is the copy of our certificate of marriage which is hereto marked as
Annex “A”;

2. That, Anthony and I were blessed with four children, namely; ADRIAN B. ANSELMO ,
ALDRIN B. ANSELMO, ANTHON B. ANSELMO and GRACE ANN B. ANSELMO.
Copies of their certificate of live birth are hereto attached and marked as Annex “B” and
series;

3. That, my children and I are applying for residency in New Zealand;

4. That, my husband is currently working as chef in New Zealand;

5. That, I used to work in New Zealand. I was here in the Philippines on vacation in 2019,
but failed to return to New Zealand when Covid-19 hit. Ergo, I currently stay here in the
Philippines to attend to our children’s needs while my husband is abroad;

6. That, I secured a certification from our Barangay evidencing that that I am the legal wife
of ANTHONY, and that we have been married since 2015. The said document is hereto
attached as Annex “C”;

7. That I am executing this affidavit to prove to attest to the truth of the foregoing
statements and for whatever legal purpose this may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 27th day of April 2023 in
