AFFIDAVIT OF LOSS

I, NENITA BETITO ORDOÑEZ, of legal age, Filipino, and a resident of 166

Trece Martirez St., San Antonio, Cavite City, Philippines, after having been duly
sworn to in accordance with law, hereby depose and state:

1. That, I am the mother of GWYN ANNE B. ORDOÑEZ;

2. That, my daughter Gwyn Anne B. Ordoñez took her Highschool and

College at Saint Joseph College;

3. That, she was graduated in the year 2000 on Highschool, and year 2003
on College;

4. That, she was issued a DIPLOMA by the above-mentioned school;

5. That, the said DIPLOMA was missing and must been lost since she
brought the above-mentioned document in other country;

6. Despite diligent efforts to locate it, the same could not be found and
beyond recovery;

7. That I am executing this affidavit to attest into the fact of loss of said DIPLOMA for
legal purposes.
