AFFIDAVIT OF UNDERTAKING

I, MARILOU CESA CAI, of legal age, married, Filipino, and a resident

of Blk. 1 Lot 7, Retirees Village 1, Mulawin, Tanza, Cavite, Philippines, after
having duly sworn to in accordance with law, do hereby depose and say:

1. That, I am an owner and proprietress of a stall located at Tanza,

Cavite;

2. That, I am earning a monthly gross income in the amount of ONE

HUNDRED FIFTY THOUSAND PESOS (Php 150,000.00) and with net
income of EIGHTY ONE THOUSAND THREE HUNDRED PESOS (Php
81,300.00) broken down as follows:

INCOME:
Monthly Income = 150,000.00

EXPENSES:
Business Employes Salary = 12,000.00
Load = 300.00
Water Bill = 400.00
Food/Drinks = 2,000.00
Expenses (Store Goods) = 50,000.00

Total Expenses: 68,700.00

NET INCOME: P 81,300.00

Business Owner of Stall on Tanza Market

3. That, the personal income that I derived from aforesaid business undertakings
is in the amount of ONE HUNDRED FIFTY THOUSAND PESOS (Php
150,000.00);

4. That, I am executing this affidavit to attest the veracity of the foregoing facts to
support my application for business/salary loan to SMARTMAB LENDING
CORP.
