SENIOR HIGH SCHOOL TEACHER II
In the exigency of the service, you are hereby designated as Youth Formation
Coordinator of Mariano Peralta National High School, Poblacion, Malita, Davao Occidental effective
immediately in addition to your regular teaching loads (references: RA 1880, CSC Resolution No.
080096, DepED Order No. 16, s. 2009, RA 9155, Magna Carta for Public School Teachers, Civil Service
Code, DepED Order No. 52, s. 2015).
In view thereof, you shall have the following functions and responsibilities:
1. Implements the mandated programs, projects, and activities on youth formation from the
Central/Regional Office/Division Office including but not limited to the Supreme Student
Government (SSG) and career guidance program.
2. Coordinates the schools on cascading the mandated programs, projects, and activities on
youth formation from the Division/Regional/Central Office.
3. Facilitates the conduct of the youth formation activities in the division level anchored on the
DepEd Youth Formation Framework and DepEd Core Values.
4. Provides inputs in the preparation Work and Financial Plans incorporating youth formation
programs, projects, and activities.
6. Develops a knowledge management system for the newly-elected SSG, and school
clubs/organizations officers and newly-appointed Teacher-Advisers and for other data-
driven youth formation activities and submits it to the Division Office.
8. Initiates other youth formation programs, projects, and activities applicable in the school.

Capacity Building
1. Modifies the training needs assessment tool for conceptualizing youth formation training
programs in the school level in coordination with the Human Resource Personnel.
5. Organizes capacity building programs for Guidance Coordinators, and School Guidance
Counselors in the conduct of career guidance program with assistance from the Director for
Academic Affairs and Director for Governance and Operations.
1. Monitors the implementation of youth formation programs including but not limited to the
school implementation of the SSG plan of actions and career guidance program.
3. Submits the report on the results of monitoring and evaluation to the Director for Governance
and Operations.
5. Recommends action research agenda based on the monitoring and evaluation results for the
improvement and development of youth formation programs.
2. Establishes partnerships and linkages in support of the youth formation programs, projects,
and activities at the school level.
