CONSENT FORM

Project Title: Baking Business

Researcher: HADIA SHAMS

Purpose of Study: The purpose of this research is to gain insights into the baking business, its
challenges, opportunities, and best practices to help small business entrepreneurs improve their
businesses. You are being asked to participate in a research study. Please read this form and ask
any questions you may have before deciding to participate.
Study Procedures: You will be asked to participate in a semi-structured interview. During the
interview, we will ask you questions about your baking business, including but not limited to,
your products, target market, marketing strategies, challenges, and successes. The interview will
last approximately 10-15 minutes.
Risks and Benefits: There are no anticipated risks associated with participating in this study.
The benefits of this study include contributing to our understanding of the baking business, and
potentially learning from other entrepreneurs' experiences.
Confidentiality: All information collected during the interview will be kept strictly confidential.
Your name and business information will not be used in any reports or publications unless you
consent to it. We will use a pseudonym to refer to you and your business in the report.
Voluntary Participation: Participation in this study is completely voluntary. You may choose
not to participate or to stop participating at any time during the interview without penalty. If you
choose to withdraw, your data will be destroyed and will not be included in any analysis.
Consent: By agreeing to participate in this study, you acknowledge that you have read and
understood this consent form, that you have had the opportunity to ask questions, and that you
voluntarily agree to participate.
