suffering from Chronic Kidney Disease (CKD)

In connection with the above subject matter, Smt Pavithra T.R, aged 34 years
(Aadhaar number 706206917125 ), wife of Shri Anil Kumar L.V. (Aadhaar
nuber :432007152499 ), aged 48 years, hereby both are residing at #23/1, T.C.
palya Main Road, Hoysala nagar, Ramamurthy Nagar, Bangalore-560016 since
last 10 years, I know them well for 10 Years.

Smt Pavithra T.R said that her husband Shri Anil Kumar L.V. is suffering from
CKD, hence the doctors at Manipal Hospital Old Airport road, Domlur
recommended for kidney transplantation where Smt Pavithra T.R agreed to
donate her One kidney to her husband voluntarily. I hereby request you to
recommend an application for consent to donate her one of the kidney as the
wife to her husband.
