Recess time MONTHLY catering service
Effective from 1st MARCH 2021, the school canteen operator will be offering the Afternoon Tea-break Service.
This service is only available for all afternoon students during their recess period.
Terms and conditions:
1. For MONTHLY catering service use this form ONLY. Parents are to be completed this form to place the
order for MONTHLY catering service.
2. A monthly fee per student will be RM3 X TOTAL NUMBER OF SCHOOL DAY IN THAT MONTH; each meal is
at RM3/student.
3. Parents are to pay the fees by every 5th day of the month. The school canteen operator reserves the right
to terminate the service immediately if the fees are not pay by the due date.
4. The school canteen operator will decide on the daily menu for the students.
5. Please inform the canteen operator if you have any food allergy problem.
6. Parents who wish to register for this monthly catering service are to complete this form and return it to
the school canteen operator together with the payment.

9. When the bio-monthly payment is due, we will issue a payment details to your child
10. For the MARCH payment as below:20days X RM3=RM60

提供 CMCO 期间下午班下课食物
由于政府还没开放学生们在食堂堂食，所以本校食堂提供打包服务，
将在 2021 年 3 月 1 日起。
条件和规则

1. 家长欲订以每月计算来订购食物必须填写此表格
2. 每位学生的月费收费以学校上学天数计算，每餐 RM3。
3. 家长必须在每个月的 5 号前付款，尚未在 5 号前缴清款项的订单则自动作废。
4. 每天的食物将由食堂经营者安排。
5. 如果学生对任何食物敏感请注明。
6. 如想订购的家长请填写表格后连同有关的收费一起交上。
7. 转账后请 WHATSAPP 学生姓名，班级和转账记录 到以上电话号码。

8. 每到收费月份，我们将发通告给予学生们。
9. 付款月份 3 月（ 20 X RM3 = RM60）
