b. One (1) Government Issued ID with signature (SSS, GSIS, Voters, Postal, Passport, Drivers License, PRC, etc.).
STRICTLY NO I.D, NO EXAMINATION;
c. Pencil No. 2;
d. Black or Blue ball pen (to be used for the full name, customary signature of the examinee and for answering the essay part in the case of PO2nd
and PO1st Class examinations);
e. Filled-out Health Declaration Form (to be provided by the NAPOLCOM Regional Office); AND PRESENT ANY of the following
(either electronic, picture, photo copy or original copy):
i. Vaccination Card/Certificate (for fully vaccinated individual); OR
ii. Reverse Transcriptase Polymerase Chain Reaction (RT-PCR) negative test result OR Covid-19 Antigen negative test result
(for non-fully vaccinated and unvaccinated individuals);
iii. Any other requirements as maybe required by the Local Government Unit concerned (if any)

WARNING :
1. Committing any of the following acts shall result in the AUTOMATIC DISQUALIFICATION from taking the examination:
