INSTRUCTIONS

1. PLEASE BE INFORMED THAT THE EXAMINEE WHOSE NAME IS INDICATED IN THIS NOTICE IS A PRIORITY 3 APPLICANT. His/her
admission to the testing rooms will only be based on the availability of examination materials.
2. PRINT this Notice and bring it together with: 1) Your vaccination card, 2) Two (2) pieces of Passport-Sized picture (3.5cm x
4.5cm) WITH NAME TAG and two (2) pieces of 2x2 picture WITH NAME TAG, and 3) One (1) VALID identification card (Any
government-issued ID, Postal ID, School ID, Passport; NBI or Police Clearance is allowed in absence of any of the former).
3. Please make sure to arrive at your exam center AT LEAST THIRTY (30) MINUTES BEFORE the session start.
4. Bring your PSA/NSO/LCR Birth Certificate (1 Original and 1 Photocopy), and your accomplished BMI Form (Original).
5. APPLICANTS WHO INDICATED "YES" ON THE INDIGENOUS PEOPLES SECTION (INDICATED HERE AS "IP DATA") ARE REQUIRED TO
SUBMIT A PHOTOCOPY OF THEIR NCIP CERTIFICATE (OR ITS EQUIVALENT) DURING THE EXAMINATION.
6. Bring medical grade facemask (surgical or N95) for emergency uses.
7. Bring your own sanitizer/alcohol.
8. Present your Notice and your valid ID to the examiner/proctor BEFORE taking the examination.
9. Bring your own #2 pencil for shading, and black ballpen (NOT SIGN OR GEL PEN) for your signatures.
10. Usage of cellular phones, calculators, or other electronic devices PROHIBITED inside the test rooms.
Unauthorized usage/possession, especially DURING the examination is an automatic ground for
DISQUALIFICATION.
11. Examinees shall sign in the applicant's attendance sheet on the examination date.
12. Room and seat assignments shall also be published in our offical Facebook page and in the test centers.
13. UNAUTHORIZED REPRODUCTION OR USE OF THIS DOCUMENT IS PUNISHABLE BY LAW.
