INSTRUCTIONS
For the Applicant:
1. Wear medical grade facemask (surgical or N95) and face shield (full
face).
2. Pass thru the foot bath with 10% chlorine solution available at the
entrance gate before entering the testing center.
3. Strictly observe physical distancing (at least 1 meter apart) in the
queue in entering the testing center.
4. All examinees shall undergo temperature scanning.
5. Only examinees with the temperature of less than or equal 37.5
degrees celsius shall be allowed to enter.
6. Bring with you alcohol (70% solution).
7. Print out (A4 size) and fill up the attached documents a) informed
consent sheet b) consent sheet, c) health declaration form and bring it
together with this NOE as proof of his/her acceptance and authority to
take the PNPACAT
8. Surrender the aforementioned documents to the Proctor before
taking the Examination.
9. Transfer of testing center is allowed under warranted situation.
10. Bring: Two (2) pieces Passport-sized ID picture, one (1) valid
Identification Card or valid Police/NBI Clearance, pencil and ballpen
(not gel pen) during the Examination.
11. If you are a member of the Indigenous People (IP), you must
provide a NCIP Certificate stating that you are a bona fide member of
your tribe.
12. Ensure that you sign the applicant's attendance sheet at the day
of the Examination.
13. Room assignment and seat number shall be published before
examination date.
