of the Philippine Consulate General in Osaka. We are pleased to
confirm the appointment schedule of the following passport

Photocopy of data page of the passport (pages 2-3)
Return envelope (120x235mm- Nagagata-3 size) with return
address of the applicant written in the center and with ¥960
worth of stamps pasted on the envelope
Exact payment of ¥7,800
Ballpen
Facemask

Additional requirements:
For first time applicants - Original plus one copy of Birth
Certificate / Report of Birth
For minors - photocopy of data page of passport of
accompanying parent
If the mother is unable to accompany the minor,
bring copy of marriage certificate
The Consulate may also require copy of birth
certificate of minor in cases where there is need to prove
filiation
If parents are unmarried, Affidavit of Support
and/or Consent executed by mother if she is cannot accompany
the child
For women changing surname due to marriage - copy of
Report of Marriage if married in Japan or
Original and 1 copy of PSA Marriage Certificate if married in
the Philippines
For women reverting to their maiden name - Original and 1
copy of Annotated PSA Marriage Certificate
(annulled /divorced by foreign spouse) or Death Certificate
(widow)

scheduled time and date. Huwag pumunta ng maaga at huwag di po
ma-late.

Everyone is required to wear a face mask when visiting the


Consulate
Everyone will be subjected to temperature checks. Those
with a fever higher than 37.5 degrees C or symptoms of a
dry cough will not be allowed entry
All clients are required to bring their own personal items
including ballpens and tissue. Borrowing of personal
items is strongly discouraged.
All clients are encouraged to wash hands with soap and
water prior to entering the Consulate’s premises
All clients are advised to practice social distancing while
inside the Consulate, following foot markers on the floor
for queues and respecting the spaces allocated between
seats
All clients are requested to avoid using their mobile phones
and reduce unnecessary conversation with other people in
the public area
All applicants will be required to disinfect their hands with
the provided hand sanitizer/ alcohol before and after
touching any Consulate equipment including fingerprint
scanners and electronic signature pens
As much as possible, applicants should bring exact change
for their payments and should place the payment on the
payment tray provided by the cashier
