STRUCTURED ON-BOARD TRAINING UNDERTAKING

This waiver of liability on Onboard Structured Training as a requirement prior to finishing the
Bachelor of Science degree is executed between: Midshipman / Midshipwoman:

I acknowledge that my 12-months/36-months On-board Structured Training is a requirement

prior to graduation and that I need to finish the Bachelor’s degree within six (6) years after
completion of classroom instructions. I further acknowledge that it carries with it the possibility of
additional refresher courses that the ASIAN INSTITUTE OF MARITIME STUDIES may impose.
The risks include; but are not limited to cost and exclusion from my original (cohorts) academic
class.

I hereby assume all of the risks of not going into an On-Board Structured Training, if the
following requirements are not complied with:

• Basic Training Certificate

• SDSD Certificate
• PASSPORT
• SEAMAN’S BOOK
• SEA SEARVICE CONTRACT
• REG. APSP ENROLLMENT
I hereby release and forever discharge and hold harmless ASIAN INSTITUTE OF MARITIME
STUDIES and its successors and assigns from any and all liability claims, as well as demands
of whatever kind or nature, in either law or equity, that arise or may hereafter arise from my not
participating in this Onboard Structured Training.

I certify that I have read this document and I fully understand its content. I am aware that this
document is a contractual release from liability of the ASIAN INSTITUTE OF MARITIME
STUDIES and that I sign it of my own free will.
