KNOW ALL MEN BY THESE PRESENT:

I, ___________________________________, the undersigned Transferee/Shifter, do hereby read,

understand and agree with the following provisions governing my admission to the Course / Program
_____________________________ in the College of __________________________________ in this University:

 That I shall submit all original documents that are required and pertinent to my admission to the
Office of the University Registrar and Admissions and/or the College
 That I shall not shift to another course once admitted
 That I shall be admitted as a probationary student for two semesters, which will commence on my
first enrollment in the University. I understand that my probationary status will only be lifted after two
semesters, provided that I obtain passing grades in all subjects enrolled during the said semesters
and I have not violated any of the rules and regulations of the College/University
 That while I am on probation, I shall report to my Guidance Counselor for guidance, follow-up, and
submission of my Preliminary, Midterm and Final grades
 That during the whole duration of my study, I shall abide by and comply with all the rules and
regulations of my College/School and the University as contained in the College Bulletin, AUF
Student Handbook, and other memorandum/communication as promulgate from time to time
 That during the whole duration of my study, I shall abide by and comply with all the rules and
regulations of the Commission on Higher Education (CHED), Technical Education and Skills
Development Authority (TESDA), and other relevant government agencies
 That I shall consistently maintain the academic excellence set by the University particularly by my
College.

In the event I violate any of the provisions of this “Undertaking”, I will voluntarily quit my schooling
with the Angeles University Foundation and/or subject myself to the sanctions prescribed by the rules
and regulations of the University.
