    SIR,

    I am Jose C. Tejares, sole heir of the late Valeriana Camacho. When my mother died she left a
    parcel of land located at Buyo, Goa, Camarines sur, covered by lot 1172-B with an area of 1,151.30 sq.m.

    When I request for a certified copy of tax declaration, said lot was erroneously declared under
    the name of Valeriana Canada, previous tax declarations reflects the correct name which is Valeriana
    Camacho, copy of tax declarations are hereto attached.

    In this regard, may I request for change and correction of name in the tax declaration from
    Valeriana Canada to Valeriana Camacho.
