1 SYSTEM OVERVIEW

O/I

Figure 1-1-1
CT 9000 ADV Contrast Delivery System

The CT 9000 ADV Contrast Delivery System is designed to inject a

radiopaque contrast medium into a patient's vascular system which
enhances diagnostic images obtained with computed tomography (CT).
Each injection is accomplished with a motor-driven syringe mechanism
with microprocessor control of the flow rate, volume, and timing.

WARNING! No modification of this equipment is allowed.

820202-J 2014-07-22 1-1-1

CT 9000TM ADV Contrast Delivery System

This manual provides operating procedures for the CT 9000 ADV Injec-
tors which includes an overview of the injector, specifications, descrip-
tions of system components, operating instructions, system generated
messages, care of equipment and troubleshooting. For further informa-
tion about service and maintenance, refer to the CT 9000 ADV Installa-
tion and Service Manual.

USER QUALIFICATION

The CT 9000 ADV Contrast Delivery System should be operated ONLY by

qualified personnel who:
• are completely familiar with the unit,
• have read and understood this Operator's Manual,
• have been trained concerning the process of how to stop an
injection in the case of an emergency (described in Chapter 5 of
this manual), and
• are otherwise properly trained in the use of equipment and
procedures of this type.

Failure to follow these guidelines could result in serious injury to the

patient or the operator.

CAUTION!
Federal law restricts this device to sale by or on the order of a physician.

SYSTEM FEATURES

VERSATILITY
The injector is microprocessor-controlled and can perform injections
within the following variable parameters:
• Flow Rate
• Volume
• Pressure Limit
• Injection Delay

INTERACTION
The operator interacts with the system by responding to prompts that
appear in the Console Display through use of eight touch screen keys.

1-1-2 820202-J 2014-07-22

INJECTION LIBRARY
A convenient, user-friendly feature of the CT 9000 ADV is its ability to
store the parameters of as many as 12 injections in a library. The names
and number of phases of all stored protocols are easily viewed in the
upper portion of the display for quick retrieval. Stored protocol infor-
mation will remain in the library even after the unit has been switched
'OFF'.

SAFETY FEATURES

The CT 9000 ADV has been designed to enhance the safety of both pa-
tient and operator. Specific safety features of the injector include:

DUAL MICROPROCESSOR
The CT 9000's dual microprocessor design provides extensive safety fea-
tures. When the unit is switched "ON," it automatically performs a series
of power-up tests to monitor the status of all injector systems. If the mi-
croprocessor detects any errors during power-up, an appropriate mes-
sage will appear in the system display. Also, during injection, all critical
injector functions are constantly checked and re-checked. If a fault is
detected, the system will automatically shut down and the appropriate
message will appear in the system display.

AIR EMBOLISM
HAZARD CHECK

DANGER! AIR EMBOLISM HAZARD!

Air entrapped in the syringe and tubing can cause patient injury or death
Always verify that both the syringe and tubing have been properly cleared
of air just prior to starting the injection! The CT 9000 ADV does not have the
capability to check for air in the syringe and tubing. The operator is respon-
sible for removing all air from the system.

The danger of air embolism is always present when dealing with viscous
contrast media. The enabling sequence for the CT 9000 ADV will prompt
the operator to check for air and the correct syringe size before an injec-
tion is delivered with the following message on the system display:
Have you evacuated all air from the XXXml syringe and tubing?

The operator should then verify that the syringe has been evacuated of
any entrapped air before proceeding.

820202-J 2014-07-22 1-1-3

CT 9000TM ADV Contrast Delivery System

START/STOP KEY ON
POWERHEAD
Because the powerhead is near the patient during an injection, the
[Start/Stop] key is integrated into the powerhead for quickly starting or
stopping an injection.

NOTE: Once an injection has been started, pressing any key on the
powerhead will stop the injection.

Remote Control Operation

An optional remote handswitch and console are available for starting or
stopping the injection.

POSITIVE POSITIONING
OF POWERHEAD
A pre-set friction device in the mounting knuckle holds the Powerhead
in the desired position during injection.

WARNING!
Collision with other equipment can cause structural failure of suspension
arm, resulting in operator or patient injury. Visually inspect the J-Bow Arm,
be sure all fasteners are secure. Observe system for cracks or defects. Check
the suspension arm for movement. If defects are observed or adjustment is
required, immediately contact your authorized service personnel.

[Forward] Key
This key is used to advance the plunger into the Syringe. Hold down the
[Forward] key while momentarily tapping one of the [Fast] keys. Holding
the [Forward] key down after the plunger has been activated will cause
the Syringe Ram to advance into the Syringe at a rate of 2 to 3 ml per
second. Tapping one of the [Fast] keys again will increase the speed of
the plunger.

NOTE: When you are using either the 100 ml or 200 ml pushrod face-
plate, continually pressing the [Fast] key in conjunction with pressing
the [Forward] key will latch plunger movement at an approximate speed
of 15 ml per second. When the scrolling dash marks on the syringe size
indicator are replaced by the syringe size value, the plunger movement
will latch "ON". You will no longer need to press the [Forward] key to
move the plunger. Latched plunger movement will automatically stop
when the plunger reaches 0 ml. To stop “latched” plunger movement,
press any key on the Powerhead.

NOTE: When using faceplates other than the 100 ml or 200 ml pushrod
faceplate, releasing the [Forward] key when the position of the ram is
greater than 5 ml will stop plunger movement. If the [Forward] key is
released when the ram position is 5 ml or less, the ram will continue to
travel until it reaches its fully extended position.

3-2-2 820202-J 2014-07-22

The [Forward] key is not functional when an injection is being delivered.
However, prior to the enabling of an injection, the [Forward] key is
functional and must not be used when the patient is attached to the
catheter. Also, prior to an injection, the [Forward] key is functional
when the faceplate is in the closed position. While the faceplate is in the
closed position, you may also turn and hold the Manual knob clockwise
to advance the plunger.

[Reverse] Key
This key is used to withdraw the ram back into the Powerhead. Hold
down the [Reverse] key while momentarily tapping one of the [Fast]
keys. Holding the [Reverse] key down after the plunger has been acti-
vated will cause the plunger to withdraw back into the Powerhead at
a rate of 2 to 3 ml per second. Tapping one of the [Fast] keys again will
increase the speed of the plunger.

NOTE: Continually pressing the [Fast] key in conjunction with press-

ing the [Reverse] key will latch plunger movement at an approximate
speed of 15 ml per second for all faceplate types used. When the scroll-
ing dash marks on the syringe size indicator are replaced by the syringe
size value, the plunger movement will latch "ON". You will no longer
need to press the [Reverse] key to move the plunger. Latched plunger
movement will automatically stop when the plunger reaches maximum
syringe volume. To stop “latched” plunger movement, press any key on
the Powerhead.

The [Reverse] key is not functional when an injection is being delivered.

NOTE: Prior to enabling an injection, the [Reverse] key is functional and

must not be used when the patient is attached to the catheter. For the
100 ml and 200 ml pushrod faceplates, the [Reverse] key is functional
only when the faceplate is in the closed position. For all other faceplates
the [Reverse] key is functional when the faceplate is in either the open
or closed position.

While the faceplate is in the closed position, you may turn and hold the
Manual knob counterclockwise to retract the plunger if needed.

CT 9000TM ADV Contrast Delivery System

[Fast] Keys
These three keys are used to increase the speed of operation of the Sy-
ringe Ram when using either the [Forward] or [Reverse] key.

While momentarily tapping any of the [Fast] keys in conjunction with

pressing either the [Forward] or [Reverse] key, the plunger will move at
a rate of approximately 2 to 3 ml per second. Continual pressure on the
[Fast] key in conjunction with pressing either the [Forward] or [Reverse]
key will cause the plunger to move at a faster rate until it reaches an ap-
proximate speed of 15 ml per second.

The [Fast] key is not functional when an injection is being delivered.

However, prior to the enabling of an injection, the [Fast] key is func-
tional and must not be used when the patient is attached to the cath-
eter. Use the Manual knob to advance or retract the plunger if needed.

[Start/Stop] Key
This key is functionally interchangeable with both the [Stop] keys and
the [Start] key on the Control Console and the optional Handswitch.
Pressing this key will start an enabled injection or stop and restart an
injection.

[Auto-Fill Mode] Key

This key is used to place the Powerhead in the Auto-Fill Mode. When the
Auto-Fill Mode is active, the Auto-Fill Indicator illuminates.

NOTE: Once in the Auto-Fill Mode, pressing any key other than the [Re-
verse] or [Fast] key will bring the Powerhead out of the Auto-Fill Mode.

[Auto-Fill Volume] Key

mL

When the Auto-Fill Mode is active, this key can be used to set a specific
fill volume for the syringe. The pre-selected volume value will be dis-
mL played on the Volume Indicator. Momentarily tap the [Auto-Fill Volume]
key to decrement the pre-selected syringe volume by 5 ml. Press and
continue to hold the [Auto-Fill Volume] key to quickly scroll through the
possible pre-selected volumes.

NOTE: The [Auto-Fill Volume] key may be used to set a fill volume with
either an empty or partially filled syringe installed. However, it is not
possible to set the fill volume to a value greater than the actual remain-
ing syringe volume.

3-2-4 820202-J 2014-07-22

NOTE: While decrementing the pre-selected syringe volume, the Vol-
ume Indicator wraps from a minimum value of 0 ml back to the maxi-
mum volume of the syringe currently in use.

Once a syringe volume has been selected, begin an AutoFill by simulta-

neously pressing and releasing the [Reverse] key and one of the reverse
[Fast] keys. The ram will withdraw into the Powerhead at a rate of ap-
proximately 8 ml per second and automatically stop when it reaches the
pre-selected volume. Tapping any key while the ram is retracting will
stop the ram and take the Powerhead out of the AutoFill Mode.

Volume Indicator
The Volume Indicator displays the volume remaining in the syringe in
real-time. Additionally, when the Powerhead is in the AutoFill Mode, the
Volume Indicator displays pre-selected volumes.

Syringe Size Indicator

mL
When the faceplate is in the closed position, the Syringe Size Indicator
displays the current syringe size being used. When the faceplate is either
in the open position or completely removed, the Syringe Size Indicator
mL displays dash marks that are continuously lit.

The Syringe Size Indicator will display scrolling dash marks when the
ram speed is less than maximum. When maximum speed is achieved,
mL
the indicator will display the current syringe size being used.

Auto-Fill Indicator
The Auto-Fill Indicator illuminates when Auto-Fill mode is ac-

5 OPERATING
INSTRUCTIONS
With the advanced design of the console and powerhead, operator
interaction with the CT 9000 ADV is greatly simplified. This chapter will
include instructions on turning on the unit, storing, deleting and alter-
ing injection parameters, syringe installation and removal procedures,
proper syringe filling procedures and injection delivery.

TURNING ON THE CT 9000 ADV

NOTE: The Injector should not be powered up with a syringe installed.

An installed syringe during power up will trigger a Used Syringe Warn-
ing message when an Injection is enabled.

With the Main power switch on, press the Console ON/OFF button
and allow the unit to display its Start-up menu (see Figure 5-1-1). From
the Start-up menu, the operator has the option of viewing one of two
menus—the Control Panel menu or the Safety menu. If neither the Con-
trol Panel key or Safety key is touched, the injector will automatically
display the Protocol Memory menu.

DELIVERING A SINGLE PHASE INJECTION—SINGLE DELIVERY *
WARNING! MINIMIZE THE POSSIBILITY OF EXTRAVASATION
Extravasation can be minimized through the following precautions:
• Use the largest vein possible.
• Use lowest flow rate practical to achieve enhancement.
• Use largest gauge teflon type catheter possible.
• Insure good backflow from catheter.
• Monitor site at least for initial portion of injection form patients side.
• Continue to monitor from remote location.
• Instruct patient to notify operator of any abnormal pain, pressure or swelling.

Refer to Figure 5-3-1.

1 Properly load and fill the syringe according to the instructions in this
Chapter.
2 1 Press the [Previous Protocol] or [Next Protocol] key to highlight
the single phase Protocol to be delivered.

2a In order to change any value of the selected protocol, press the

[Change Values] key. For information about changing values, follow
Steps 1—3 of Changing Values of an Existing Protocol. After changing the
values, press the [Enable] key. (The changed values will not be stored in
memory.) Or…

2b If the protocol values are not to be changed, press the [Enable] key.

If a syringe is currently loaded, the following message will appear on the

console.

WARNING!
Air may be present
Syringe was not changed after
last injection. Please install new
syringe to continue

Replace the syringe to continue with an injection.

NOTE: This message will not appear if the used Syringe Warning system
has been deactivated. Contact a qualified service technician to re-en-
able this feature in order to reduce the risk of injecting patients with air,
which could result in patient injury or death.

* When using the 125 prefill syringe the volume remaining display on the console automatically updates
for the prefilled syringe chosen. The power head display will agree with this display as long as the prefill
selection was made while the ram was within 13 ml of full capacity. If the prefill selection is made
with the ram in a position more than 13 ml from maximum the Powerhead will display the remaining
volume from 125 ml regardless of the prefill selection. The console will always display the remaining volume
updated for the prefill selected and will not allow an attempted injection with insufficient volume remaining.

STOPPING AN INJECTION

An injection may be immediately halted by pressing any of the follow-

ing keys. While in the pause mode, the console will beep every minute
and will also display the elapsed time of the pause.

POWERHEAD
Press the [Start/Stop] keys on the Powerhead once to immediately stop
an injection. Refer to Figure 5-3-7.

NOTE: Once an injection has been started, pressing any key on the
powerhead will stop the injection.

To restart the injection, press either the [Start/Stop] key on the

Powerhead, the [Start] key on the Console or the Remote Handswitch.

CONSOLE
Refer to Figure 5-3-8. Pressing any key on the console will "Stop" the
injector. All eight keys provide a signal to software to Stop (Pause) an
injection. The bottom key additionally is hardwired to the main micro-
processor to stop an injection. Due to its redundant nature, the bottom
key should be considered the "Primary" Stop key.
To restart the injection, press either the [Start/Stop] key on the
Powerhead, the [Start] key on the Console or the Remote Handswitch.

NOTE: During a stopped (paused) injection, time will continue to accu-

mulate under the Total Time display. Also, an audible "beep" will occur
every minute.

REMOTE HANDSWITCH
If using a Remote Handswitch, fully press down once to immediately
stop an injection. To restart the injection, press either the [Start/Stop]
key on the Powerhead, the [Start] key on the Console or the Remote
Handswitch.

TERMINATING AN INJECTION

As shown in the previous section, any key on the Powerhead and the
[Stop] keys on the Console and the Remote Handswitch can all be used
to pause or stop an injection. These keys are recommended to be used
to stop an injection in the case of an emergency.

The least recommended method to stop an injection is to press the

Console's On/Off button. Using the Console On/Off button to stop an
injection will terminate the injection and turn off the unit. If an injec-
tion is stopped by the use of this button, the injection must be re-en-
abled. The achieved values will also be lost.

INSTALLING THE 200 ML FRONT LOADING SYRINGE

A 200 ml Front Loading syringe is designed to fit into the mating pres-
sure sleeve. This syringe contains an umbrella cap that secures onto the
pressure sleeve and allows for aseptic insertion of the syringe into the
pressure sleeve. Both sleeve and syringe are translucent to permit easy
visual inspection of the remaining volume.

NOTE: If you install a syringe when the ram is positioned at 10 ml or

less, the injector will automatically move the ram to the 0 ml position.

WARNING! INFECTION HAZARD!

Contamination of sterile-packed syringes is possible if proper precautions are
not taken. Due to the risk of serious patient infection, do not touch the
interior of the syringe or expose the interior of the syringe to conditions
which can cause contamination (coughing, sneezing, etc.) Immediately place
opened syringe into injector sleeve to prevent contaminating the interior of
the syringe.

Refer to Figure 5-4-2.

1 Run the ram to the fully extended position.

2 1 Rotate the Pressure Plate lever to the Load position.

2 Using aseptic technique, remove the syringe from its sterile packaging
by grasping the umbrella cap. Insert the syringe into the sleeve by
matching the Load Arrow on the syringe to the Load Arrow on the
pressure sleeve. The syringe can now slip down into the sleeve.

NOTE: If you lock the syringe in place while the ram is positioned at 10
ml or less, the injector will automatically move the ram to its fully ex-
tended position.
3
Lock the syringe into place by either turning the Pressure Plate lever to
the Lock position or by rotating the umbrella cap clockwise until the
load arrows align as shown. If the lever cannot be moved to the Lock po-
sition, make sure the Load Arrows are aligned properly, then gently
push down on the umbrella cap. The lever should now lock easily

FILLING THE 200 ML FRONT LOADING SYRINGE
WARNING! INFECTION HAZARD!
Contamination of sterile-packed syringes is possible if proper precautions are
not taken. Due to the risk of serious patient infection, do not touch the
Linden-Luer adapter, the section of a fill tube that will be inserted into the
contrast, or the inside of the syringe unless sterile gloves are worn.

DANGER! AIR EMBOLISM HAZARD!

When filling syringe, check connecting line and syringe to ensure that air is
not being sucked into the system due to a leaking fitting. Air entrapped in
the line can cause patient injury or death.

NOTE: Preheating the contrast medium will help in the removal of air
bubble.

1 Tilt the Powerhead so that the syringe is pointing straight up

2 Remove the protective cap from the syringe tip.

3 Using aseptic technique, slide the end of the shorter section of the fill
tube over the syringe tip. DO NOT TOUCH THE TIP. Place the end of the
longer section of the fill tube into the container of contrast media. To
keep aeration to a minimum, verify that this end of the tube is in the
medium and is NOT drawing air.

Manual Fill Technique

4 Press and hold the Reverse key and tap the Fast key once to slowly draw
the desired volume of contrast medium into the syringe. Refer to Figure
5-4-3 and 5-4-4. Press the Forward key and tap the Fast key to expel air
from the syringe or use the instructions contained in the Section—Re-
moving Air Bubbles from the Syringe. Filling the syringe too fast will
cause undesired aeration of the contrast.

AutoFill Technique
Activate the AutoFill mode and establish a pre-selected syringe volume
(See definitions for [Auto-Fill Mode] and [Auto-Fill Volume] keys in Chap-
ter 3).
NOTE: When using the AutoFill Technique, add an extra 5 ml to 10 ml to
the desired syringe volume (i.e., pre-selected syringe volume) to com-
pensate for volume losses due to tubing volume and mechanical toler-
ances inherent to the injector.

820202-J 2014-07-22 5-4-5

CT 9000TM ADV Contrast Delivery System

Once an AutoFill syringe volume has been established, press and release
the Reverse key and the Fast key simultaneously. The syringe plunger
will automatically draw the pre-selected volume of contrast medium
into the syringe. Press the Forward key and tap the Fast key to expel air
from the syringe or use the instructions contained in the Section—Re-
moving Air Bubbles from the Syringe. Filling the syringe too fast will cause
undesired aeration of the contrast.

5 To minimize spillage of contrast on the injector, leave the level of con-

trast just at the tip of the syringe or slightly below or leave the level in
the fill tube slightly below the tip of the syringe.

6 Attach extension tube, remove air by either pressing the Forward key or
by using manual knob on the Powerhead

7 Secure extension tube in the slot of the pressure plate lever until ready
for use. Refer to Figure 5-4-5. Just prior to injection, the Powerhead must
be rotated so that the syringe is below horizontal with the syringe tip
pointing downward throughout the actual injection.

NOTE: Using the fill keys may temporarily prevent the Total Time from
being displayed. However, Total Time will continue to accumulate in the
background.

REMOVING AIR BUBBLES FROM THE SYRINGE
DANGER! AIR EMBOLISM HAZARD!
Air entrapped in the syringe can cause patient injury or death. Always verify
that the syringe and tubing has been properly cleared of air!

Air bubbles entrapped in contrast media that has been drawn into a
syringe MUST be removed to prevent injection of an air embolism into
the patient. Such bubbles may be most readily removed by performing
the following.

1 Remove the heater blanket in order to allow an unobstructed view of

the contents of the syringe and to prevent damage to the heater blan-
ket while trying to dislodge the air bubble.

2 Draw an extra 20 cc of air into the syringe to form an air pocket; this will
provide a "washing action" for removing air bubbles. Wait for any air
bubbles entrapped in the contrast to rise to the top (outlet end of the
syringe).

3 Slowly rotate the powerhead through an 150° arc once or twice to allow
the air pocket to pick up the air bubbles on the sides of the syringe. Re-
fer to Figure 5-4-6. To dislodge any stubborn air bubbles, tap the syringe
with the heel of the hand.
CAUTION! DO NOT STRIKE THE HEATER BLANKET!
Do not strike the heater blanket when dislodging air bubbles. Irreparable
damage to the heater blanket can occur. Remove the heater blanket prior to
dislodging air bubbles.

4
Advance the plunger to remove the air pocket from the syringe by ei-
ther pressing and holding the Forward key while tapping the Fast key or
by slowly rotating the manual control knob clockwise

5
With the air bubbles removed, the Powerhead should remain in the
vertical position until ready for injection.

The syringe may be removed by either turning the Pressure Plate lever
to the Load position or by rotating the umbrella cap counterclockwise
until the load arrows align as shown. The syringe can be easily lifted out.

NOTE: The syringe may be removed without disconnecting the exten-

sion tube to minimize spillage of contrast and to save time.

DANGER! REMOVE SYRINGE PROMPTLY

Failure to remove syringe after completion of a procedure can lead to an
inadvertent injection of air. Used syringes should be promptly removed from
the injector immediately after a procedure is completed to avoid accidental
re-use of the empty syringe. Injecting air can cause patient injury or death.

INSTALLING THE IV POLE (OPTIONAL)

Refer to Figure 5-4-21

1 Mount the IV pole base, with console bracket (if used), to the stand
assembly as illustrated in Figure 5-4-20. Verify that the restraining pin at
the top of the stand assembly passes through the smaller holes in the
washer, console bracket (if used), and IV pole base.

NOTE: The label on the IV pole base should face upward.

CAUTION!
The bracket and base are not yet secure to the remote stand assembly and
could fall, possibly causing damage to the equipment. Continue to support
the bracket and base until the Powerhead is in place.

2 Lower the Powerhead onto the stand assembly by passing its stem
through the larger hole in the IV pole base, console bracket (if used),
washer and finally into the larger central clearance hole in the remote
stand assembly. Tighten the securing screw to keep the Powerhead in
place.

NOTE: As the Powerhead is lowered into place, the restraining pin on

the remote stand assembly will pass upward into a mating socket drilled
into the shoulder of the pivot.

3 Tighten the securing screw to keep the Powerhead locked to the stand
assembly

4 Insert the IV pole through the hole in the IV pole base. Lock the IV pole
into place by tightening the locking knob.
DANGER!
Ensure that the locking knob is secure before loading the IV pole. Failure to
completely tighten the locking knob can result in IV pole tipping, catheter
site trauma, and patient injury.

NOTE: When not in use, the locking knob can be removed and the IV
pole reattached upside-down for storage.

DANGER!
Do not place more than 10 lbs. (4.5 kg) on the IV pole. Exceeding this
weight limit can result in IV pole tipping, catheter site trauma, and patient
injury.

7 CARE OF UNIT

DAILY INSPECTION

PRESSURE SLEEVE
Check the pressure sleeve daily for signs of stress, crazing lines, or cracks
and replace immediately when any of these symptoms are found.

WARNING! CHECK PRESSURE SLEEVE FREQUENTLY!

Syringe pressure jackets must withstand pressures generated during injec-
tion delivery. Defective sleeves may shatter or explode under these condi-
tions. Always inspect pressure sleeve closely before using injector. Rotate the
pressure sleeve while viewing all areas; look for stress cracks (around the
front or at the shoulder area), discard any pressure sleeve exhibiting signs of
stress, crazing lines or cracks. The use of such parts may cause injury and/or
an aborted injection.
