AFFIDAVIT

I, REMIGIO GONZALES CASTRO, of legal age, Filipino and a

resident of San Jose, Baliwag, Bulacan, under oath depose and say:

1. That I am the same REMIGIO GONZALES CASTRO who

was born on October 20, 1951 at Baliwag, Bulacan and co-
owner of a parcel of land located at San Jose, Baliwag,
Bulacan with an area of 525 square meters;

2. That, when I secured my Transfer Certificate of Title No. TC-

4000 issued by Registry of Deeds of Bulacan, it turns out
that my name was erroneously encoded/entered as
“REMEDIOS CASTRO” instead of “REMIGIO GONZALES
CASTRO” as indicated in my Driver’s License (No. C07-79-
030184);

3. That the names “REMEDIOS CASTRO” in the said

Transfer Certificate of Title and “REMIGIO GONZALES
CASTRO” in the said ID, refers to one and the same person;

4. That the discrepancy between my Transfer Certificate of

Title and ID is due only to a clerical error;

5. That I am executing this affidavit to attest the truth of the

foregoing facts and in inform the authorities concerned about
the same and for legal intents and purposes.
