AFFIDAVIT

I, JOYCE ERICKA LOPEZ CRISTOBAL, of legal age, Filipino

and a resident of Matangtubig, Baliwag, Bulacan, under oath depose
and say:

1. That I am the same JOYCE ERICKA LOPEZ CRISTOBAL,

born on July 16, 1996;

2. That, when I secured my Investigation Data Form issued by

Office of the Provincial Prosecutor, it turns out that my name
was erroneously encoded/entered as “JOYCE ERICA
CRISTOBAL y LOPEZ” instead of “JOYCE ERICKA
LOPEZ CRISTOBAL” as indicated in my UMID ID (No.
0111-9294339-1);

3. That the names “JOYCE ERICA CRISTOBAL y LOPEZ” in

the said Investigation Data Form and “JOYCE ERICKA
LOPEZ CRISTOBAL” in the said ID, refers to one and the
same person;

4. That the discrepancy between my Records and Document &

ID is due only to a clerical error;

5. That I am executing this affidavit to attest the truth of the

foregoing facts and in inform the authorities concerned about
the same and for legal intents and purposes.
