AFFIDAVIT OF BIRTH ORDER

I, ALBERT CASTA CELZO of legal age, Filipino citizen, a resident of No. 24 Mindoro
St. Batasan hills, Quezon City, after having been sworn to in accordance with law hereby
depose and say:

1. My Seventh Daughter CHARITY CANONIZADO CELZO was born on 20TH day of

JULY 2010 as appearing on her Certificate of Live Birth attached as “ANNEX A”;

2. However, the birth order of my Seventh Daughter CHARITY CANONIZADO

CELZO, born on 20TH day of JULY 2010, was erroneously entered as “FIFTH” as
appearing on her Certificate of Live Birth attached as “ANNEX B”;

3. Wherefore, I am executing this Affidavit to attest to the truth of the foregoing

facts as a requisite of my PAG-IBIG Fund Provident Claims and for the correction
of my Seventh Daughter birth order.
