I. Objectives:

a) Understand the purpose and importance of accurately filling out various forms.
b) Identify and comprehend the different components of common forms, such as school forms,
deposit slips, and withdrawal slips.
c) Develop the necessary skills to fill out forms accurately, including neat handwriting, following
instructions, and providing correct information.

II. Subject Matter:

Topic : Fill-out forms accurately (school forms, deposit, and withdrawal slips, etc.)

Materials : Sample forms (school enrollment form, deposit slip, withdrawal slip), Worksheets with,

2. Motivation

Direction: Find out the image by completing the pieces of jigsaw puzzles.

4. Discussion

Begin the lesson by discussing the importance of filling out forms accurately. Explain to students that
forms are used for various purposes, such as gathering information or completing transactions.
Emphasize that accuracy is crucial because incorrect or incomplete information can cause problems or
delays. Present examples of different forms commonly encountered by students, such as school
enrollment forms, deposit slips, and withdrawal slips. Engage students in a brief discussion about the
similarities and differences between the forms.

Group Activity: Filling Out Forms Practice

(Distribute worksheets with form templates for practice.)

Instruct students to work individually or in pairs to fill out the forms based on the provided
instructions. Walk around the classroom to provide guidance and support as students complete the
forms. Encourage students to double-check their work for accuracy, neatness, and completeness.
Go to their respective groups fill out the following forms assigned to each group.
