APPENDIX

We are Lower Six students of Presentation College, San Fernando. Our names are Kymani
Mc Phie, Akash Nimchan, Alex Paul, Aaron Maharaj, Justin Raghoonanan and Rayall Narine.
This questionnaire will be used to determine the positive effects of sport participation on
the mental health of students ages 15-19 from x to y. To do this we will be collecting
statistical data from students form x-y. The data collected will be used to complete our
Caribbean Studies IA. Furthermore, we aim to gain a better understanding of this topic
through the analysis of said data. We assure you that your responses are anonymous and
confidential. The survey is broken up into 5 parts; personal information, stress, self-esteem,
socialization and mood. Besides the personal information, definitions of unfamiliar terms are
included at the top of each page to ensure that respondents understand the questions
better.

affecting nearly every system of the body, influencing how people feel and behave.
