3+4 Home Quarantine Regulation Statement

I (hereinafter referred to as Party B) in accordance with the Central Epidemic Command Center's
regulations, all incoming travelers who enter Taiwan MUST implement Home-Quarantine which
includes 3-days Home Quarantine and 4-day Self Epidemic Prevention. During Home quarantine
period at Cho Hotel/Cho Hotel 2, Party B has acknowledged and agree to abide to the following
Terms and Conditions. Cho Hotel/Cho Hotel 2 (hereinafter referred to as Party A) reserves all the
rights to manage or reject any reservations.

1. For those who choose to pay to Party A directly, a Credit Card Pre-authorization of the total
accommodation fee 7 days before the check-in date is required for securing the reservation.
Party B will need to pay the Total Accommodation Fee 4 days after check-in either by
Remittance or Credit Card (Cash, Union Pay and American Express are NOT accepted). Party B
MUST check-out before the designated time at the end of quarantine (11a.m.) and make sure
to pack all personal belongings before check-out. Party A is not responsible for keeping any
items left behind by Party B and the staff of Party A reserves the right to discard items left
unattended by Party B.

2. If Party B is unable to check-in according to schedule due to the POSITIVE result on PCR testing
upon arriving, there won’t be any cancellation fee once Party B provides the PCR positive result
as proof. If Party B is notified by the Government officials regarding the POSITIVE result on
PCR testing after Party B has already checked in at Party A. The room rate will be charged
accordingly: as the latest check-out time is 11 a.m., shall Party B checks out before 3 p.m., there
will be an extra charge of NT$200/hr.; shall Party B checks out after 3 p.m., Party B will be
charged one night’s room rate.

3. Party B requires to take the body temperature in the morning and evening and submit the
online form “Daily Body Temperature Record” on daily basis. Party B shall truthfully state the
temperature measurement information. Party B shall be responsible for any misstatement that
causes Party A’s misreporting; Party A is not responsible for verifying the record submitted by
Party B. If Party B shows any symptoms of COVID-19, Party B is responsible for informing Party
A immediately. Party A will assist Party B in seeking medical assistance.

4. Party A will Only provide all delivery service at the 3 designated times (Breakfast 09:00~10:00,
Lunch 12:00~13:00, Dinner 18:00~19:00). Any delivery NOT delivered within the designated
time, then Party A will deliver all items over the next designated delivery time. Party B shall
settle all payment of any delivery services that he/she orders in advance. Party A will NOT
settle the payment for any delivery on behalf of Party B.

5. Party A is NOT allowed to enter Party B’s room during the quarantine period for any reasons.
Party B MUST keep the room clean. In-room garbage should be bagged and placed inside the
 garbage can outside the door from 15:30 to 16:00 every day by Party B. Party A will collect the
garbage and sanitize it every day.

6. Party B CANNOT leave the room without any official approval or under any emergency
circumstances such as earthquake. In case of emergency, Party B MUST wear mask to evacuate.
Visitors and deliveryman are strictly forbidden to enter Party B’s room. Any gatherings with
other hotel guests are Strictly Prohibited in quarantine hotel. Party B can open the room door
Only for receiving deliveries and throwing garbage away and MUST wear a mask. If Party A
finds any violation of Party B, Party A will report to the health authorities and terminate Party
B’s accommodation. According to CDC latest 3+4 Home Quarantine Program: Shall Party B
needs to go outside during the 4-day Self-Epidemic Prevention for Work or For Purchase
Necessities, Party B MUST provide the NEGATIVE result of COVID Rapid Test within 2 days and
complete the “Self-Epidemic Prevention Outgoing Application” form prior the outgoing day.
Party B should also sign and follow the “Outgoing Regulation Agreement” issued by Party A.

7. Party B shall comply with below hotel policy during the stay at Party A: (1) It is forbidden to
use any higher power electronics and private power supply in the room for safety purpose. (2)
Party B is liable for any damage caused (whether deliberate, negligent, or reckless act) to the
room or any assets at Party A’s property during Party B’s stay. (3) Do not make noises to
disturb the comfort and peace of other guests. (4) No pets allowed. (5) According to Tobacco
Hazards Prevention Act, smoking is strictly prohibited. A fine ranging from NT$2,000 to
NT$10,000 will be imposed if smoking inside the rooms. If the fire alarm goes off or there is a
fire caused by smoking, Party B should be responsible for any damage and loss caused to the
hotel and the other hotel guests. (6) Any illegal acts such as drug use and theft will be reported
to the police immediately. (7) Party A reserves the right to open all delivery that consist of
suspected items such as alcoholic drinks, lighters, cigarettes, and any sharp objects such as
knives to Party B and to hold it in care until Party B’s check-out date; Or to reject any delivery
that includes all the above items.

I agree to abide by the above "Cho Hotel/ Cho Hotel 2 Home Quarantine Regulation
Statement," and Party A reserves the right to terminate Party B’s accommodation at
any time if Party B is in full or part breach of this provision.
