Student during the 2nd semester of the school year 2021-2022 is officially enrolled in Child &
Adolescent Literature taken during the said school year.

Furthermore, the above named of student has incurred a remark of “87” for the reason of
incomplete course requirements. However, Ms. TIMBASAL had already complied the
deficiencies that he incurred in the said course, and is given a grade of “92”.

The undersigned therefore respectfully requests your good office to facilitate the grade of
the said student.
