SPECIAL POWER OF ATTORNEY

Norte, Philippines, do hereby name, constitute and appoint ________________________, of legal age, Filipino, and
resident of _________________________, Tagum City, Davao del Norte, Philippines, to be my true and lawful
Attorney-in-Fact for my name, place and stead:

1. I am the Vendee of a parcel of land more particularly described as _________________________;

2. I was appointed by the Vendor as an Attorney-in-Fact by virtue of a document denominated as Deed

of Sale with Assumption of Mortgage and Special Power of Attorney,

3. I do hereby name, constitute and appoint ____________, to do the following acts as enumerated in the
above-mentioned document, to wit:

a. To represent and to act for and in his behalf in transacting business with the Pag-IBIG Fund
or any government agency relative to his Real Estate Mortgage;
b. To cause the issuance of Cancellation of Mortgage upon full payment of the mortgage
indebtedness and to receive the owner’s copy of the __________________________ from the
mortgagee Pag-IBIG Fund or it’s assignee;
c. To apply / transact with water, power, telephone and other utilities;
d. To execute, sign, deliver any contract, documents or papers of whatever kind necessary to
accomplish the above-mentioned purposes.

4. Furthermore, I appoint ___________________ to process, transact, and negotiate with BIR, ROD, DAR, LGU
and any other government agency pertaining to the transfer of registration of the above-described

purposes as I might or could lawfully do if personally present and hereby ratifying and confirming all that my
