I am applying for TEACHER 1 at JANTIANON ELEMENTARY SCHOOL, JANTIANON, AMLAN

NEGROS ORIENTAL. That I understood well the DepEd Memorandum No. 61,s.2022 of pertinent papers for
Elementary School.

b) CSC Form 212 (Revised 2005) in two portion with latest 2x2 picture
c) Certified photocopy of Authenticated PRC License
d) Certified photocopy of Authenticated Rating in LET/PBET
e) Certified Photocopy of Transcript of Records
f) Copies of Service Records, Performance Rating and School Clearance ( For those with Teaching
Experience)
g) Certificates of Specialized Training, if any Certified copy of Voter's ID and/or any proof of
residence as deemed acceptable by the School Screening Committee
h) NBI Clearance
i) Omnibus Certification of Authenticity and Veracity of all documents submitted and signed by the
applicant

2. I am executing this Certification to attest to the authenticity and veracity of all documents submitted.
3. I understand that any violation will automatically disqualify me from the selection process.
