By virtue of the powers in me by Republic act. 7160, otherwise as the

Local Government Code of 1991, you are hereby appointed as RESCUE
MEMBER of Barangay Mabayuan, Olongapo City effective effective upon
taking your oath of office and to serve in such capacity with all the rights,
privileges and responsibilities appointment thereto until the end of the term
of the appointing authority unless sooner replaced.

The herein appointee is not related to the undersigned to the fourth degree of
affinity or consanguinity.
