Effective today, January 10, 2019, you are hereby assigned as AMBULANCE DRIVER in lieu of
MR. REYNOLO LABARDA.
You shall report to work Monday to Sunday, 5:00 AM to 5:00 PM, for this assignment. You
are directed to conduct preventive maintenance check-up every after duty and shall assist the
Municipal Mechanic in the event of repair or replacement of ambulance spare parts.
Coordinate with Dr. Lourdes Calzita, Municipal Health Officer, for further instructions.

Greetings!
The Local Government Unit of Barugo, Leyte, through the Sanggutan Festival contingent, has been
invited to participate and represent the Philippines in the up and coming Chingay Parade at
Singapore. This Chingay Parade is considered as one of the famous and most-awaited cultural event
in Singapore which showcases unique cultural presentations coming from different regions.
Considering its large and international audience, the Chingay Parade will serve as our ticket to
introduce to the world our very own Sanggutan Festival, which in turn, will help boost our cultural
tourism.
After the conduct of auditions for Sanggutan Festival dancers, we discovered that we have
insufficient number of Sanggutan Festival dancers. To compensate the lacking number of dancers,
we opted to solicit the participation of talented teachers in our Municipality to be part of the
Sanggutan Festival contingent.
Relative thereto, may we respectfully request for your permission to allow the following teachers
and/or students to join and participate the Sanggutan Festival contingent to present in the Chingay
Parade in Singapore on February 13 – 19, 2019 as well as to allow them to practice and perform
preparatory activities connected thereto, to wit:
Furthermore, may we respectfully for the facilitation and immediate release of FORM 137 of the
abovementioned students as of the documentary requirements for PASSPORT application.
